Health intake consent
A working consent statement for collecting health information and providing assessment-led bodywork.
01Purpose of the intake
Therapy by Dee proposes collecting relevant health information so an authorised therapist can assess suitability, identify contraindications, adapt treatment, and support a safer session. This page is a draft statement only and does not record consent.
02Information you provide
The intake may ask about symptoms, pain, mobility, pregnancy, allergies, medications, recent procedures, medical conditions, injuries, and other facts relevant to treatment. Clients should answer accurately and update the therapist if circumstances change before or during a session.
03Sensitive information consent
The production form is expected to request explicit, specific consent to collect, use, securely store, and share necessary health information with the assigned therapist for treatment and safety. Consent wording, withdrawal handling, and any legal exceptions require review under applicable UAE law.
04Treatment is not medical care
Therapy by Dee services are not a substitute for diagnosis, emergency care, or treatment by a physician or other regulated medical professional. Clients should seek appropriate medical advice for concerning symptoms and disclose any professional restrictions or guidance.
05Clearance and contraindications
Written surgeon clearance is required for post-operative drainage, together with the approved minimum interval after the procedure. Medical clearance may also be requested for post-caesarean scar work, first-trimester prenatal treatment, or any condition where treatment suitability is uncertain.
06During the session
Clients may ask questions, decline a technique, change pressure, or stop the session at any time. Therapists may modify, postpone, or stop treatment if information, symptoms, conduct, or environmental conditions create a safety concern.
07Storage and access
Health intake and clearance records are expected to be encrypted, access-restricted, and separated from general booking notes. The approved retention period, hosting location, authorised roles, deletion process, and privacy contact must be added before production use.
08Consent confirmation
The production flow must include an unticked consent control, the final approved wording, document version, timestamp, and a method to withdraw consent where legally available. No consent should be inferred from viewing this draft page.
